Job Summary
The Oncology Case Manager plays a crucial role in supporting patients diagnosed with cancer through their treatment journey. This position emphasizes improving patient outcomes by coordinating care, facilitating access to resources, and providing essential support to both patients and their families.
Responsibilities
- Assess patient needs and develop individualized care plans to address challenges related to their cancer treatment.
- Coordinate with healthcare providers to ensure seamless delivery of services and continuity of care.
- Provide patient education regarding treatment options, side effects, and supportive resources.
- Act as a liaison between patients, families, and medical professionals to facilitate communication.
- Monitor patient progress and adjust care plans as necessary to meet evolving needs.
Qualifications
- Bachelor’s degree in Nursing, Public Health, Social Work, or a related field; RN or LCSW preferred.
- Proven experience in oncology or case management is highly desirable.
- Strong communication and interpersonal skills to effectively engage with patients and families.
- Knowledge of cancer treatment protocols and patient navigation is beneficial.
- Ability to work independently and as part of a multidisciplinary team.
