**Job Title: Registered Nurse Case Manager**
The Registered Nurse Case Manager delivers comprehensive and personalized care to patients within their homes and facilitates end-of-life discussions. This role includes crafting care plans, symptom management, and working alongside a multidisciplinary team to ensure optimal patient outcomes. Applicants should have an Associate, Bachelor’s, or Master’s degree in nursing and a current RN license. A minimum of one year of recent experience in acute care is necessary, as well as dependable transportation and a valid driver’s license.
