Job description
**Job Title: RN Case Manager**
The RN Case Manager delivers direct nursing care while assessing patient requirements and coordinating services in line with the established care plan. Key responsibilities include maintaining accurate clinical documentation, overseeing nursing staff, and facilitating patient transitions, which encompass discharges and recertifications. Applicants should have graduated from an accredited nursing program and possess a minimum of one year of clinical nursing experience. A valid Indiana RN license is mandatory, alongside experience in managing the medical and behavioral needs of clients who are medically fragile.
