Job description
The RN Case Manager is responsible for coordinating and ensuring quality, cost-efficient care for patients with ESRD and CKD. This position involves working closely with healthcare professionals and family members to manage personalized care plans and assist patients during challenging healthcare transitions within a designated area. Applicants should possess an Associate Degree in Nursing (A.D.N) or a higher qualification, hold a valid RN license, and have a minimum of five years of clinical nursing experience. Additionally, candidates must demonstrate proficiency in case management, possess basic computer skills, and have a valid driver’s license.
