Job description
**Job Title: RN Case Manager**
The RN Case Manager is responsible for performing initial admission evaluations and orchestrating patient care strategies to guarantee effective management of clinical resources. They play a key role in planning for patient discharge and act as a bridge between patients and the multidisciplinary healthcare team. Applicants should possess a nursing degree and be actively working towards a Bachelor’s degree if they have an Associate’s degree. A current Kentucky nursing license or a Nurse Licensure Compact (NLC) license is mandatory.
