The RN Case Manager performs nursing assessments and coordinates hospice care for patients in their homes. They work with an interdisciplinary team to develop and implement care plans while ensuring compliance with regulatory standards. Candidates must have at least two years of clinical nursing experience and one year of full-time hospice experience. A current state Registered Nurse license and a valid driver’s license are required.
