**Registered Nurse Case Manager**
The Registered Nurse Case Manager is responsible for conducting patient evaluations and working alongside the healthcare team to create and execute tailored care plans. This role includes overseeing the activities of LPNs and HHAs while fostering patient autonomy through educational initiatives and rehabilitation strategies. Applicants should possess an active RN license and a minimum of one year of clinical experience in the nursing field. Additionally, a valid driver’s license, dependable transportation, and up-to-date CPR certification are necessary qualifications.
