Job Summary
The RN Case Manager plays a crucial role in providing expert nursing services directly in patients’ residences. This position involves creating and overseeing personalized care plans in partnership with a multidisciplinary team.
Responsibilities
- Perform comprehensive assessments of patients’ health needs.
- Develop and manage individualized care plans tailored to each patient.
- Deliver patient education to enhance understanding and compliance.
- Ensure precise and thorough clinical documentation to support optimal health outcomes.
Qualifications
- Graduation from a state-recognized nursing program.
- A valid Registered Nurse (RN) license.
- Minimum of one year of relevant nursing experience.
- Possession of a valid driver’s license and auto insurance.
- Current Basic Life Support (BLS) certification.
- Completion of mandatory reporter training as required.
