Job Summary
The RN Case Manager is responsible for delivering skilled nursing services directly to patients in their homes. This role includes conducting thorough assessments, formulating care strategies, and offering educational support to patients regarding their health.
Responsibilities
- Perform skilled nursing assessments in patient homes.
- Develop and implement individualized care plans tailored to patient needs.
- Provide education to patients and their families on health management.
- Collaborate effectively with an interdisciplinary team to coordinate holistic patient care.
- Supervise Licensed Practical Nurses (LPNs) and home health aides to maintain high standards of patient care.
Qualifications
- Active and unrestricted Registered Nurse (RN) license.
- Current driver’s license.
- Valid CPR certification.
- At least one year of professional experience as an RN in home health or acute care settings.
