The case manager is responsible for evaluating the psychosocial requirements of patients and their families to create and execute thorough discharge strategies. They will also be involved in crisis intervention, provide counseling, and act as a bridge between patients and community support services. Applicants are required to possess a Master’s degree in Social Work (MSW). Although having 0-1 years of experience is acceptable, a preference will be given to candidates with at least one year of experience in hospital-based case management.
