**Job Title: Social Worker Case Manager**
The Social Worker Case Manager is responsible for assessing patient requirements, coordinating discharge planning efforts, and managing transitions within the entire care continuum. This role involves serving as a bridge between patients, their families, and the multidisciplinary healthcare team to promote effective care management and address conflicts. Applicants are required to have a Master’s Degree in Social Work (MSW) and a minimum of one year of experience in clinical care or case management. Additionally, candidates must obtain a Basic Life Support (BLS) certification prior to beginning their employment.
