Job Summary
The Social Worker plays a vital role in delivering compassionate support to hospice patients and their families, focusing on psychosocial, emotional, and economic requirements. This position involves collaboration with an interdisciplinary team to formulate comprehensive care plans, conduct home consultations, and facilitate end-of-life arrangements.
Responsibilities
- Provide supportive services to hospice patients and their families to meet diverse needs.
- Work closely with the interdisciplinary team to create and implement care plans.
- Conduct home visits as part of patient assessments and support.
- Assist families with arrangements related to end-of-life care.
Qualifications
- A Master’s degree in Social Work is required.
- Active licensure in Georgia and Alabama is mandatory.
- Clinical experience in a relevant field is essential.
- Strong communication skills are necessary to effectively interact with patients and families.
- The ability to maintain a flexible schedule is important, including participation in on-call rotations.
