Job Summary
The Hospice Social Worker plays a crucial role as the expert in psychosocial support within an interdisciplinary team. This position focuses on delivering assessments, counseling, and creating care plans tailored for patients and their families. The Social Worker will also assist in navigating resources, educating families on matters related to end-of-life care, and ensuring the emotional and social wellness of individuals during their hospice experience.
Responsibilities
- Conduct psychosocial assessments for patients and their families.
- Provide supportive counseling to address emotional and social needs.
- Collaborate on care planning within the interdisciplinary team.
- Assist families in accessing necessary resources and services.
- Educate families regarding end-of-life care options and implications.
- Maintain awareness of the emotional and social well-being of patients throughout their hospice care.
Qualifications
- Master’s Degree in Social Work (MSW) from a CSWE-accredited program.
- Valid state licensure for social work practice.
- A minimum of one year of experience in a healthcare or hospice setting.
- Excellent communication skills and cultural sensitivity.
