Job Summary
This role is dedicated to carrying out detailed in-home health evaluations for elderly patients and those at risk, aiming to detect chronic health issues and enhance overall health results. The position involves working closely with primary care physicians, family members, and caregivers to achieve synchronized care and maintain accurate records.
Responsibilities
- Conduct comprehensive health assessments in patients’ homes.
- Identify chronic health conditions affecting older adults and at-risk individuals.
- Collaborate with healthcare teams, including primary care physicians and caregivers, to provide coordinated care.
- Document health findings clearly and effectively for ongoing patient management.
Qualifications
- Master’s degree in Nursing or a related field.
- Current Nurse Practitioner licensure with prescriptive authority.
- Board certification in a relevant area of specialty.
- Up-to-date certifications in BLS, ACLS, or CPR.
