Job Summary
The Social Worker plays a crucial role in delivering direct care management and social work services tailored to PACE program participants. This position emphasizes promoting independence and enhancing the quality of life for individuals involved in the program.
Responsibilities
- Conduct assessments to evaluate the needs of participants.
- Facilitate interdisciplinary team meetings to ensure comprehensive care coordination.
- Coordinate care through effective counseling and community referrals.
Qualifications
- A Master’s degree in Social Work.
- A valid state license to practice as a social worker.
- Preference for candidates with a minimum of two years of experience in clinical or community-based environments.
- At least one year of experience working with the elderly population is advantageous.
