**Job Title: Hospice Social Worker**
The Hospice Social Worker acts as the psychosocial support specialist within an interdisciplinary team, conducting evaluations, developing care plans, and offering counseling to both patients and their families. Additionally, they provide education on hospice services, help families navigate available resources, and keep track of psychosocial developments during the end-of-life process. To qualify, candidates must have a Master’s Degree in Social Work (MSW) from a CSWE-accredited institution and maintain current licensure in their state. At least one year of experience in a healthcare or hospice environment is essential.
