The social worker will provide acute medical social work services and manage discharge planning for patients in a hospital setting. Responsibilities include maintaining a caseload of approximately 1:25 and coordinating care to ensure effective patient transitions. Candidates must hold a Master of Social Work (MSW) degree and possess at least two years of experience in acute care social work and discharge planning. A valid state licensure and BLS certification are required for the position.
