**Social Services Specialist**
The Social Services Specialist is responsible for recognizing the social, emotional, and psychological needs of patients and their families to enhance their overall well-being. This role involves working closely with a multidisciplinary team to formulate care plans, oversee discharge processes, and maintain adherence to regulatory requirements. Applicants should have a bachelor’s degree in social work or a related human services discipline. Furthermore, they are required to have any necessary state certifications or licenses to practice in long-term care settings.
