**Job Title: Targeted Case Manager**
The Targeted Case Manager oversees every facet of a youth’s case, which encompasses evaluating needs, developing service plans, and tracking progress. This role involves working closely with families and service providers to address intricate needs while ensuring precise record-keeping. Applicants should be at least 21 years of age and hold a bachelor’s degree in a human services discipline or have relevant experience. Successful completion of a background check and the capability to type 35 words per minute in an electronic medical record system are mandatory.
