**Job Title: Team Manager**
The Team Manager is responsible for guiding the Patient Care Team by overseeing, assessing, and coordinating the efforts of interdisciplinary team members to deliver exceptional care. This position also involves acting as an advocate for patients and ensuring detailed patient records are maintained throughout the intake process, discharge, or transfer. The ideal candidate will be a skilled leader adept at managing diverse teams in a hospice setting. Strong organizational abilities and a dedication to the organization’s mission concerning end-of-life care are essential qualities for applicants.
