Job Summary
The Telehealth Registered Nurse (Tele RN) position involves delivering exceptional patient care through remote health services. The candidate will utilize their nursing expertise and technological skills to monitor, assess, and provide support to patients virtually, ensuring their health needs are met efficiently and effectively.
Responsibilities
- Conduct remote patient assessments and health monitoring through telecommunication platforms.
- Develop personalized care plans based on individual patient needs and health conditions.
- Educate patients about health management, treatments, and medication adherence.
- Collaborate with healthcare teams to ensure continuity and quality of care for patients.
- Document patient interactions and care provided accurately in medical records.
Qualifications
- Valid Registered Nurse (RN) license in the relevant state.
- Proficient in using telehealth technology and electronic health records.
- Strong communication skills with a patient-focused approach.
- Experience in nursing, preferably in a telehealth or remote care setting.
- Ability to handle multiple cases while maintaining attention to detail.
