Job Summary
The Transplant Med-Surg/Tele/SDU position is pivotal in safeguarding the recovery and ongoing health of patients who have undergone transplantation procedures. This role involves providing specialized nursing care within a dynamic medical-surgical, telemetry, and step-down unit environment. Nursing professionals in this capacity are responsible for closely monitoring patient conditions, implementing care plans, and coordinating with interdisciplinary teams for optimal patient outcomes.
Responsibilities
- Monitor and assess the health status of post-transplant patients, identifying any changes in their condition.
- Administer medications and manage intravenous therapies as per physician orders.
- Collaborate with multidisciplinary teams to create and execute comprehensive care plans tailored to individual patient needs.
- Educate patients and their families regarding recovery processes, medication management, and necessary lifestyle adjustments post-transplant.
- Document patient care activities accurately to ensure continuity and quality of care.
- Respond promptly to emergencies and complications that may arise during patient recovery.
Qualifications
- A valid and active nursing license in the state of practice.
- Experience in transplant nursing, preferably in a Med-Surg or Telemetry environment.
- Strong communication skills, both verbal and written, to facilitate effective collaboration.
- Critical thinking and problem-solving abilities to manage complex patient situations.
- Basic life support (BLS) and advanced cardiac life support (ACLS) certifications are required.
