Job Summary
The Transplant Med-Surg/Tele/SDU position is vital in delivering specialized care to patients undergoing transplant procedures and managing their post-operative recovery. The role focuses on providing comprehensive medical-surgical and telemonitoring services within a supportive healthcare environment, ensuring high-quality patient outcomes through effective assessment, intervention, and collaboration with healthcare professionals.
Responsibilities
- Monitor patient health remotely and in-person, using advanced telemonitoring technologies.
- Manage assessments and care plans for patients before and after surgical transplants.
- Collaborate with interdisciplinary teams to ensure seamless patient care and recovery processes.
- Provide education and support to patients and their families regarding treatment plans and health management.
- Document patient progress and care interventions accurately in medical records.
- Respond promptly to changes in patient conditions and implement appropriate interventions.
Qualifications
- Active nursing license with experience in surgical, tele, or critical care settings.
- Strong understanding of transplant procedures and post-surgical management.
- Excellent communication and interpersonal skills for effective patient and team engagement.
- Able to interpret cardiac rhythms and perform relevant medical interventions.
- Certifications in ACLS and BLS may be required.
