**Job Title: Family Nurse Practitioner**
Engage in virtual wellness consultations, perform health evaluations, and review medications for both adult and pediatric patients. Work closely with in-home clinical teams to ensure seamless care coordination, manage referrals, and accurately document health assessments. Candidates must hold a Board Certification as a Family Nurse Practitioner and possess a valid Texas nursing license. This role calls for excellent communication abilities, proficiency with technology, and a skillful balance of independent and team-oriented work.
