Job Summary

- Oversee a caseload that averages around 1:25, ensuring attentive management of each patient’s needs.
- Coordinate comprehensive care plans to facilitate effective transitions from hospital to home or other care settings.
Qualifications
- A Master’s degree in Social Work (MSW) is mandatory.
- At least two years of professional experience in acute care social work and discharge planning is essential.
- A valid state licensure is required to practice.
- BLS certification must be maintained throughout employment.
