Job Summary
The Navigator Social Worker is responsible for delivering psychosocial assistance to patients and their families while enhancing care coordination through a collaborative transdisciplinary approach. This position focuses on evaluating emotional and financial requirements, crafting care plans, and supporting discharge processes along with facilitating access to community resources.
Responsibilities
- Provide psychosocial support to patients and their families.
- Facilitate care coordination within a multidisciplinary team.
- Assess emotional and economic needs of patients.
- Develop and implement comprehensive care plans.
- Assist with the discharge planning process.
- Help patients acquire community resources.
Qualifications
- Must hold a valid social work license in the State of Ohio.
- At least one year of experience in a recognized agency or healthcare organization.
- Possession of a valid driver’s license, reliable transportation, and auto insurance.
