Job Summary
The Hospice Registered Nurse will carry out detailed assessments of patients and formulate tailored care plans in conjunction with the interdisciplinary team. This role entails coordinating patient care, delivering approved treatments, and offering both emotional and clinical support to patients along with their families.
Responsibilities
- Perform thorough patient assessments to ascertain individual needs.
- Collaborate with the interdisciplinary team to create personalized care plans.
- Coordinate overall patient care and manage prescribed treatments.
- Provide emotional and clinical support to patients and their families.
Qualifications
- Graduate of an accredited Registered Nurse program.
- Possession of an active South Carolina or Compact RN license.
- A minimum of one year of nursing experience is required; experience in hospice care is preferred.
