Job Summary
We are seeking a dedicated School Based Health Center Nurse Practitioner or Physician Assistant to join our team at Well Care Community Health in Austin, Indiana. This position is primarily onsite and offers a variety of healthcare services to students, ensuring their health and well-being in a school setting. Responsibilities
- Conduct assessments and screenings for students within the school-based health center.
- Provide laboratory services and CLIA waived testing as necessary.
- Collaborate with school personnel to address the health needs of students.
- Maintain accurate medical records and documentation of patient encounters.
- Educate students and families about health-related issues and preventive care.
Qualifications
- Valid Nurse Practitioner or Physician Assistant certification and state licensure.
- Experience in a primary care or school health setting is preferred.
