Job Summary
The Nurse Practitioner or Physician Assistant in Palliative Medicine will be engaged in providing specialized patient care with a focus on managing complex health issues and improving the quality of life for patients with serious illnesses. This position is located onsite in Phoenix, Arizona, and involves collaboration with a multidisciplinary team including Physicians, Palliative Care Nurses, and Social Workers.
Responsibilities
- Conduct comprehensive health assessments to evaluate patients’ needs.
- Collaborate with healthcare professionals to create and implement effective care plans.
- Facilitate shared decision-making regarding patient treatment options.
- Maintain adherence to the principles of Advanced Practice Registered Nursing and the ANA Code of Ethics.
- Document patient interactions and care plans accurately in medical records.
Qualifications
- Advanced clinical skills and capabilities in assessing patient health.
- Certification as a Nurse Practitioner or Physician Assistant.
- Strong communication skills for effective collaboration with the healthcare team.
- Understanding of palliative medicine principles and practices.
