Job Summary
The primary focus of this role is to deliver empathetic and direct personal care along with mobility support to hospice patients, whether in their residences or care facilities. This position is essential in ensuring the comfort and well-being of patients during their final stages of life.
Responsibilities
- Provide personal care and assistance with mobility to hospice patients in their homes or designated care settings.
- Observe and document any changes in patients’ conditions, reporting these observations to the registered nurse.
- Assist patients with daily living activities to enhance their quality of life.
Qualifications
- Completion of approved training as a hospice aide, home health aide, or nurse aide, including demonstrated competency.
- Possession of relevant state-specific certifications during the course of employment.
- Ability to travel within the designated service area to meet patient needs effectively.
