**Care Coordinator**
The Care Coordinator works in partnership with the Behavioral Health Team to evaluate patient needs, execute care strategies, and deliver continuous support and advocacy. This role involves overseeing patient interactions, ensuring precise documentation, and coordinating care with external community and behavioral health services. Applicants should possess a bachelor’s degree in a clinical behavioral health discipline and have a minimum of two years of experience in case management or care coordination. Bilingual proficiency in Spanish and English is essential, as well as the capability to thrive in a dynamic, collaborative setting.
