Job Summary
The role involves overseeing the patient discharge and transition processes, ensuring that patients move smoothly from acute care to post-acute care settings. This includes working closely with various multidisciplinary teams, medical professionals, and community resources. The position serves as the main contact for both patients and their families during this critical transition.
Responsibilities
- Coordinate patient discharge and transitions with a focus on effective communication among teams.
- Act as the primary liaison for patients and their families to facilitate seamless post-acute care transitions.
- Collaborate with healthcare professionals and community resources to ensure comprehensive care post-discharge.
Qualifications
- High School Diploma or GED is required, with a preference for candidates holding an Associate’s degree.
- Certification in Basic Life Support (BLS) is mandatory.
- Prior experience in a healthcare environment is essential.
