Job description
The Care Manager is responsible for community outreach aimed at connecting with individuals experiencing homelessness and delivering trauma-informed, person-centered assistance. Key duties involve conducting assessments, handling crises, and facilitating care by collaborating with and referring to existing support services. Applicants should have a Bachelor’s degree in a relevant discipline or hold a New York State Licensed Practical Nurse license, with considerable case management experience accepted as a substitute. Essential qualifications include excellent interpersonal skills, the ability to support marginalized groups, and adeptness in crisis de-escalation techniques.
