Job Summary
The Care Manager Supervisor plays a crucial role in overseeing and supporting Care Managers to deliver exceptional health home services. This position involves ensuring that all activities are compliant with regulatory standards and that documentation is accurate and up-to-date. The supervisor will also focus on the development of staff to empower individuals in realizing their health and life objectives.
Responsibilities
- Provide oversight and guidance to Care Managers in delivering quality health services.
- Monitor documentation for accuracy and compliance with regulatory requirements.
- Facilitate professional development and training for Care Managers.
- Assist individuals in achieving their health and life goals.
- Travel across Upstate NY as necessary to fulfill job duties.
Qualifications
- Minimum of one year of supervisory experience.
- Bachelor’s degree with at least two years of relevant experience, or a Registered Nurse license with two years of practice, or a Master’s degree with one year of experience.
- Proficiency in health record technology.
- Ability to travel throughout Upstate NY.
