Job Summary
The Care Manager plays a crucial role in coordinating and overseeing patient care services to ensure that individuals receive optimal support and resources. This position involves working closely with a diverse team to facilitate patient needs and enhance their overall health outcomes.
Responsibilities
- Develop and implement individualized care plans tailored to each patient’s requirements.

Qualifications
- Degree in healthcare, social work, or a related field.
- Experience in case management or a similar role is preferred.
- Strong communication and interpersonal skills to engage effectively with patients and their families.
- Ability to work collaboratively in a multidisciplinary team environment.
- Familiarity with healthcare regulations and best practices in patient care.
