Job Summary
The Care Manager plays a crucial role in coordinating and overseeing patient care services to ensure that individuals receive optimal support and resources. This position involves working closely with a diverse team to facilitate patient needs and enhance their overall health outcomes.
Responsibilities
- Develop and implement individualized care plans tailored to each patient’s requirements.
- Collaborate with healthcare providers, patients, and families to ensure comprehensive care delivery.
- Monitor and evaluate the effectiveness of care plans, making necessary adjustments as required.
- Provide education and resources to patients and their families regarding available support services.

Qualifications
- Degree in healthcare, social work, or a related field.
