Job description
**Care Manager**
The Care Manager is tasked with conducting professional evaluations of client capabilities and developing tailored service plans to meet their needs. This role includes overseeing the well-being of clients, keeping accurate case documentation, and championing the interests of the senior demographic. Applicants should live in close proximity to the designated area of service and possess a valid license as a social worker, registered nurse, or licensed practical nurse. Alternatively, candidates with a bachelor’s degree in a health and human services discipline coupled with two years of pertinent experience will also be considered.
