**Job Title: Case Manager**
The Case Manager plays a vital role in assisting individuals facing homelessness by offering support and making referrals to promote self-sufficiency and stable housing. Key responsibilities encompass performing assessments of clients, developing tailored case plans, and ensuring accurate documentation within the HMIS database. Ideal candidates should hold an associate degree in a social or behavioral science discipline, complemented by a minimum of one year of experience in social services. Additionally, applicants must have a valid Arizona driver’s license and the capability to secure a Level One Fingerprint Clearance Card.
