**Job Title: Case Manager**
The Case Manager oversees patient care from the point of admission to discharge, ensuring that treatment is both medically sound and cost-efficient. This role involves working closely with a diverse team of professionals to evaluate patient requirements, manage resource utilization, and promote a safe transition throughout the entire care process. Applicants are required to possess a Bachelor of Science in Nursing along with a current Registered Nurse license in Illinois. A minimum of one year of experience in case management within an acute hospital environment is also necessary.
