Job Summary
The Field Care Manager Nurse (RN) plays a crucial role in delivering high-quality care coordination for patients in various settings. This position involves engaging with patients to assess their healthcare needs, developing care plans, and ensuring effective follow-up to promote optimal health outcomes.
Responsibilities
- Conduct thorough assessments of patient needs and conditions.
- Design and implement individualized care plans based on assessments.
- Coordinate with healthcare providers to facilitate care and services.
- Monitor patient progress and adjust care plans as necessary.
- Educate patients and their families about health management and resources.
- Document care activities and patient interactions accurately.
- Participate in team meetings to discuss patient care strategies.
Qualifications
- Active Registered Nurse (RN) license.
- Experience in case management or field nursing preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time efficiently.
- Familiarity with electronic health records (EHR) systems.
- Knowledge of community resources and healthcare services.
