Job description
**Job Title: Case Manager**
The Case Manager delivers focused, field-oriented assistance to adults experiencing homelessness who are grappling with mental health issues, medical concerns, and substance use disorders. Key responsibilities encompass outreach efforts, assisting with housing application processes, coordinating care services, and ensuring precise electronic health record maintenance. Applicants are required to hold a bachelor’s degree in an appropriate discipline alongside a minimum of one year of relevant experience, or two years of hands-on experience in mental health and permanent supportive housing. A valid driver’s license for California and successful completion of fingerprint clearance are necessary prerequisites.
