Responsibilities include developing collaborative treatment plans, coordinating care, and ensuring members are linked to housing, medical, and psychiatric services. Candidates must have a Bachelor’s degree in Social or Human Services with one year of experience, or a high school diploma with four years of direct mental health experience. A valid driver’s license, personal vehicle, and insurance are required for client transportation.
