**Job Title: Case Manager**
The Case Manager is responsible for conducting outreach and engagement initiatives aimed at individuals with behavioral health challenges, guiding them towards treatment options. This role also involves supporting clients in navigating community resources and developing as well as executing personalized treatment plans. Applicants should possess a bachelor’s degree in a human services discipline or have three years of applicable experience, and must hold or be eligible for certification as a Qualified Mental Health Associate (QMHA). A current driver’s license, along with the capacity to successfully complete background and DMV checks, is essential.
