Job Summary
The Certified Home Health Aide plays a crucial role in delivering personal care and mobility support to clients in their residences, adhering to established care plans. This position also involves monitoring client health conditions, offering companionship, and ensuring precise documentation of provided care.
Responsibilities
- Deliver personal care and assistance with mobility to clients following their care plans.
- Monitor and observe clients’ health conditions to ensure their well-being.
- Provide companionship to clients to enhance their emotional support.
- Maintain meticulous records of the care administered and changes in client conditions.
Qualifications
- Must possess a current New Jersey Certified Homemaker-Home Health Aide (CHHA) certification.
- Must exhibit compassion and dependability in caring for clients.
- Ability to adhere to and implement individualized care plans effectively.
