Job description
**Job Title: Care Management Specialist for Children**
This role involves delivering direct care management to children suffering from chronic illnesses through home, school, and community visits. Responsibilities include performing thorough assessments, creating individualized care plans, and facilitating medical and community support services for clients. A Bachelor’s or Associate’s degree in Human Services or a similar discipline is required, along with a minimum of two years of relevant experience. Applicants must have a valid NYS driver’s license, access to a dependable vehicle, and be proficient in Microsoft Office applications.
