The Clinical Referral Coordinator is responsible for overseeing complex clinical referrals for home health and hospice services, serving as a crucial link between referral sources and the agency. This role includes ensuring precision in documentation, streamlining the admissions process, and upholding exceptional standards of patient care and customer satisfaction. Ideal candidates should have graduated from an accredited nursing program and possess a valid Registered Nurse (RN) license. A minimum of one year of experience in home health or hospice settings is highly desirable.
