Job Summary
The counselor plays a vital role in delivering outpatient mental health therapy and conducting evaluations for patients and their families, whether in individual or group contexts. This position emphasizes creating comprehensive treatment plans while ensuring meticulous maintenance of medical records and effective collaboration with referring healthcare providers.
Responsibilities
- Provide outpatient mental health therapy and evaluations to individuals and families.
- Develop and implement personalized treatment plans for patients.
- Maintain accurate and up-to-date medical records for all clients.
- Coordinate patient care with referring providers to ensure comprehensive support.
Qualifications
- A Master’s degree in a relevant field is required.
- Must hold an active LCSW or LCPC license in Illinois.
- Experience in providing psychotherapy within a clinical environment is essential.
- Familiarity with electronic medical record (EMR) systems is necessary.
