Job Summary
The Health and Wellness Coordinator LVN is responsible for overseeing health programs aimed at promoting wellness and ensuring quality patient care within the organization. This role involves collaboration with various teams to enhance health initiatives and support the overall health goals of the community served.
Responsibilities
- Develop and implement wellness programs that address the needs of the community.
- Coordinate health-related activities and services, ensuring compliance with health regulations.
- Provide direct patient care and education regarding health maintenance and disease prevention.
- Monitor patient outcomes and adjust programs as necessary to improve effectiveness.
- Work alongside healthcare professionals to assess and meet the health needs of individuals and groups.
Qualifications
- Valid Licensed Vocational Nurse (LVN) certification.
- Experience in health education or program coordination preferred.
- Strong communication skills and ability to work in a team-oriented environment.
- Knowledge of community health resources and wellness strategies.
