Job Summary
The Home Health Field RN is responsible for delivering extensive nursing care, conducting assessments, and developing treatment plans tailored to patients receiving care in their homes. This role involves working closely with interdisciplinary teams and offering education to patients and their families to promote seamless care and achieve the best possible health outcomes.
Responsibilities
- Deliver comprehensive nursing care and perform patient assessments in home settings.
- Develop and implement individualized treatment plans based on patient needs.
- Collaborate with various healthcare professionals to ensure coordinated care.
- Educate patients and their families about health management and treatment options.
- Monitor patient progress and make necessary adjustments to care plans.
Qualifications
- Current Registered Nurse (RN) license.
- Valid driver’s license.
- CPR certification.
- Preferred experience of at least 2 years in home health care.
- Proficiency in using HomeCare HomeBase software.
