**Job Title: RN Case Manager**
The RN Case Manager delivers personalized skilled nursing services to clients in their residences while guiding the formulation and execution of care plans focused on achieving specific objectives. This role also involves offering clinical direction to team members, ensuring adherence to documentation protocols, and facilitating collaboration with the wider healthcare team. Applicants must hold a valid Registered Nurse license in Minnesota and possess a minimum of two years of experience in either a healthcare or home care environment. Additionally, a current driver’s license, dependable transportation, and successful completion of background checks are essential.
