Job description
The RN Case Manager will create and modify care plans for patients in collaboration with a diverse team that includes LPNs, therapists, and doctors. This role entails performing home visits, conducting OASIS assessments, and offering education to patients and their families. Applicants must have a current RN license in Oklahoma as well as a CPR certification. Although experience in home health is preferred, candidates with a solid assessment background from other clinical settings are also encouraged to apply.
