Job Summary
The Home Visiting Nurse – Family Connects is a crucial role within the healthcare sector, focusing on providing essential nursing services to families in Monmouth County. The position emphasizes delivering quality care and support to new parents and their infants, ensuring a smooth transition into parenthood. Responsibilities
- Conduct home visits to assess the health and needs of mothers and their newborns.
- Provide guidance and education on infant care, parenting, and maternal health.
- Establish supportive relationships with families, fostering a sense of trust and security.

- Document visit details and maintain accurate health records for each family.
Qualifications
- Registered Nurse (RN) license in the state of New Jersey.
- Experience in maternal and child health or community health nursing is preferred.
