Job Summary
The role involves delivering personal care and emotional support to patients facing terminal illnesses in either their residences or care centers. The individual in this position will ensure that comprehensive records of patient interactions are maintained and will communicate any alterations in the patient’s physical or mental health to the relevant care team members.
Responsibilities
- Provide direct personal care to terminally ill patients, catering to their physical and emotional needs.
- Maintain thorough and accurate documentation of patient visits to track progress and care provided.
- Notify the care team of any significant changes in the patient’s condition, whether physical or mental.
- Work collaboratively with patients’ families and healthcare providers to ensure comprehensive support.
Qualifications
- An active Certified Nursing Assistant (CNA) license is required.
- Current CPR certification is mandatory for employment.
- A valid driver’s license is essential, along with access to a reliable vehicle.
- Applicants must possess at least six months of hands-on experience in direct patient care.
